FALL LEAGUE - Forms

  • Post-season Survey Form - please complete by November 15
  • Game Report Form - required for U12 games; optional for U10 games
U12 teams must submit the game report by 9pm on Monday following each game; otherwise a $5 fine is assessed.

  • Team Registration Form and payment - due July 24, 2013  
    This is a paper form that needs to be printed, completed and delivered to the Fall League Coordinator
    by the deadline.

    Late applications accepted through August 3, 2013 by 12 noon with $25 added late fee. 
  • Team Scheduling Request Form - due July 24, 2013; no exceptions!
This is an online form that needs to be completed by the coach or manager by the deadline.

  • Field Availability Form - due August 4, 2013 by 12 noon; no exceptions! 
This is an online form that needs to be completed by the club Field Coordinator by the deadline. 
  • Code of Conduct Form - Due September 3, 2013
    This is a paper form that needs to be printed, completed/signed by the coach(es) and manager, and delivered to the Fall League Coordinator by the deadline.  $10 fine if late, plus it MUST be received along with NYSWYSA roster prior to first game.

  • Game Re-schedule Form - Grace period 8/9-8/14 only

NOTE:  Game changes after the grace period ends are limited and must be requested via email to TompkinsCountyFallSoccer@gmail.com.  I.f you do not receive a reply within 24 hours, please contact the TCSFL Coordinator by phone.
