REGISTRATION FORMS

Youth Soccer Programs in the TCS area:
Youth Soccer and Adult Soccer are delineated at the age of 19. A player who turns 19 years of age before August in any given year is considered a "U20" player and is thus an Adult player and cannot participate in local Club soccer programs. A player who turns 19 years of age on or after August 1 of any given year is considered a "U19" player and is still eligible to participate in local Youth Club soccer programs.

Not sure what age group your player can register for? Check out the age matrix.

New players and parents in the Lansing area that have an interest in soccer should visit our web page that describes the differences between Recreational and Competitive Soccer. and review our registration instructions that cover every season.

Find a team (or start one yourself!) and register!

Below are links to various LSC forms. Review our page of Registration Instructions.

Player Registration ........Player Medical Release............Player Secondary Roster Form

Parents and Players: These forms are required of players playing on one of the LSC indoor or outdoor teams. If you are the parent of a player you should download the LSC Player Registration form, as well as the LSC Player Medical Release form . For new players to the club a photocopy of either the players Birth Certificate or Passport is required. Print out these forms, fill them in and give them to the head coach or manager of the team along with the registration fee of $15.00 for full membership or $8.00 for associate (secondary) membership. Associate members must already be registered with NYSWYSA through another affiliated soccer club (ask the coach or manager how they want the check made out). If your team is registering in a league that requires player passes there is an additional fee of $6.00 for a 'Primary Team' pass and $6.00 for a 'Secondary Team' pass, you will need to provide a 1" picture of the player; your coach or manager will let you know if a pass is required. Player passes are not required in the Winter League for indoor soccer.

Team Roster.................Team Staff Registration..............

Coaches and Managers: If you are an LSC coach or manager you must obtain a NYSWYSA Risk Management Pass for yourself. in addition to the forms for your individual players you will also need the LSC Team Roster form, and the LSC Team Staff Registration form. Spring teams only: for players playing on a second team they will need to fill out the LSC Player Secondary Roster form (this is not required for Winter League Indoor Soccer).
