Fill out a registration and medical release form for your child and give it to the team manager

Still need help? Email Lansingsoccer@gmail.com and we will be happy to help you.

To find out what age group your player can register for check out the age matrix. Generally a player can play "up" an age bracket but they can not play "down" an age bracket. Youth Soccer and Adult Soccer are delineated at the age of 19. A player who turns 19 years of age before August in any given year is considered a "U20" player and is thus an Adult player and cannot participate in local youth Club soccer programs. A player who turns 19 years of age on or after August 1 of any given year is considered a "U19" player and is still eligible to participate in local Youth Club soccer programs.

Risk Management: A background check done by NYSWest Youth Soccer Association.
Any adult who will be working with Lansing Soccer Club soccer players as a manager, coach or assistant coach ARE REQUIRED to have a risk management pass. If you need a risk management pass, email the club registrar, Sarah Lange, at lansingregistrar@gmail.com and include your FULL name, Birthdate, home address, home phone number plus the email address you would like to use (cell and work numbers as well as a fax number are encouraged but optional to include). The registrar will then enter your data into Roster Pro and NYSWYSA will email you instructions; please respond asap. The Registrar will receive your pass and notify you. As a service to our members the LSC will pay for your risk management pass; the cost is $10 and is good for two years.

Below are links to various LSC forms. Review our page of Registration Instructions.

FORMS: 1. Player Registration ........2.Player Medical Release...........3.Player Secondary Roster Form (for associate members only)

Parents and Players: These forms are required of players playing on one of the LSC indoor or outdoor teams. If you are the parent of a player you should download the LSC Player Registration form, as well as the LSC Player Medical Release form . For new players, that have never registered with NYSWest before, a photocopy of either the players Birth Certificate or Passport is required to confirm the player's age. Print out the club registration forms, fill them in and give them to the head coach or manager of the team along with the registration fee of $15.00 for full membership.

Associate members are players that are already registered with NYSWYSA through another affiliated soccer club; they would fill out a Player Secondary Roster Form and pay $8.00 for associate (secondary) membership(ask the coach or manager how they want the check made out).

If your team is registering in a league that requires player passes there is an additional fee of $6.00 for a 'Primary Team' pass and $6.00 for a 'Secondary Team' pass, you will need to provide a 1" picture of the player; your coach or manager will let you know if a pass is required. Player passes are not required in the Winter League for indoor soccer.

Team Roster.................Team Staff Registration..............

Coaches and Managers: If you are an LSC coach or manager you must obtain a NYSWYSA Risk Management Pass for yourself (see procedure in our registration instructions). in addition to the forms for your individual players you will also need the LSC Team Roster form, and the LSC Team Staff Registration form. Spring teams only: for players playing on a second team they will need to fill out the LSC Player Secondary Roster form (this is not required for Winter League Indoor Soccer).
