OBTAINING A COACH’S
PASS
Each adult who plans to be on the same sideline as the soccer
players during a game needs to have a coach’s pass.
This includes the coach, assistant, and any other helpers.
A coach’s pass is obtained by agreeing to a background
investigation. This is done to help insure the safety of the
players. The referee may ask for it at any time and ask you
to leave the sideline if you don’t have your pass with
you.
In order to obtain a pass, go to the New York State West
website at www.nyswysa.org
and link to Risk Management. There you will see a disclosure
form that you need to print out. Fill in the form and mail
it to the address provided at the bottom of the form, along
with a $6.00 check made payable to NYSW. The form itself does
not indicate that there is a fee, but the cost per pass is
$6.00. When your pass expires, you are required to re-submit
a new disclosure form and pay the fee again.
There are several portions of the form that need clarification:
1. Our district is Binghamton (not Ithaca)
2. Do not check any box under “program” unless
you know that it applies to you. Most of our applicants will
leave it blank.
3. It is important for you to fill out the bottom section
entitled “Passes shall be returned to”. Club official
is Karen LaCelle. Club program is Lansing Soccer Club. Club
address is 645 Salmon Creek Road, Lansing, NY 14882.
Coach’s passes will be mailed to me, and I will forward
them to you. Please add your picture and laminate per the
