Team Registration Form – Deadline: July 25

• Form and fee (must submit together) are due to the League Coordinator by July 25. Late registrations accepted July 26 thru August 1 with additional $25 late fee.
• If you do not receive an email confirmation of your registration by August 1, please immediately contact the League Coordinator BY PHONE at 607-533-7940. Teams who submit their Team Registration and Scheduling Request forms earliest will receive priority for scheduling requests.

Team Scheduling Request Form – Deadline: August 1

• Form must be received by August 1; otherwise, don’t even bother sending it (any conflicts will have to be handled during the grace period)

Field Availability Form – Deadline: August 1

• Form must be received by August 1. REQUIRED, and it should be one form per home field.

Code of Conduct Form – Deadline: September 1

• A signed Code of Conduct must be received by September 1 along with a NYSWYSA team roster
• A $10 penalty will be assessed to the team if these two documents are not received by the deadline.
• Teams may NOT play a league game until the Code of Conduct and NYSWYSA roster is received by the League Coordinator.
• Coaches & manager signatures only (players need not sign)
